ADC false readings


hello, i'm working on project feed voltage a0 using op295 op amp. op amp give me steady voltage of lets 1.30v, counts should @ 266, adc read +-20-30 counts off, 220 or 290, , read different @ given time. using default reference voltage , seems steady during duration of analogread. when try feed a0 pin external power supply @ 1.30 volts, counts should be. can't seem figure out problem here. have placed traditional .1uf caps vcc gnd, , a0 gnd. never seem able achieve value should at. appreciated. thanks.
